Social Support Group

SOCIAL OUTING

This program provides a social outing for those who meet the eligible criteria; it also enables carers to have a break. The service picks up clients each weekday from all over the Wollondilly and Camden LGA’s. Activities are arranged in consultation with the clients and vary from day to day. A quarterly activity program is distributed giving client’s details of activities and outings. This may include barbeques, shopping trips, guest speakers, picnics, concerts and themed days. Morning tea and lunch are provided in the fixed based facility or centre based settings. The groups do not meet on public holidays. Staff and volunteers are on hand to give any assistance needed. Our buses are fitted with a wheelchair hoist.

SPECIAL EVENTS

There are times when the group attends a special event; for example, a cruise or restaurants – you may be requested to pay for the cost of the event prior to the day. This is because the service may have to book and pay for a certain number of guests and unfortunately some venues require full payment even if not all guests attend. Because of this there could be times when you pay for a function and may not receive a refund if you are unable to attend. Receipts for weekly contributions will be issued upon request. Additional monies collected in advance for special functions, donations etc. will be automatically receipted.

Clients are asked to contribute toward the cost of meals and transport provided to them. Individuals are also responsible for personal costs if the group is attending a venue where there is an entrance fee, meals or some other fee involved.

REFERRALS AND ELIGIBILITY

Aged

People can be referred through the MyAgedCare gateway on www.myagedcare.gov.au or just call 1800 200 422. Clients will be assessed to be included in Commonwealth Home Support program or a Home Care Package: frail older people and younger people with a disability and their carers. Eligibility assessment of potential clients is carried out by the regional assessment team in your area.

People with a disability

  1. Check your eligibility by completing an NDIS checklist on the Am I
    eligible page.
  2. Make a verbal application by calling 1800 800 110 or complete an
    Access Request Form and return it to the NDIS.
  3. Get your evidence of disability, functional impact, and residency
    status.
  4. Wait for the NDIS to contact you and confirm your eligibility and
    funding plan.
